What is Case Management?
· Case Management for Children and Pregnant Women (CPW) is a Medicaid service provided by a case manager to help children and pregnant women and their families access the services the child and/or pregnant woman needs.
Who is eligible for Case Management Services?
· Children (ages 0-21) and pregnant women on Medicaid with a health condition or at risk for a health condition are eligible to receive case management for children and pregnant women.
· Pregnant women and infants (age 0-1) who qualify for Title V with a health condition or at risk for a health condition are eligible to receive case management services.

· Health Issues
· Identifying and making referrals in the community
· Advocating for clients
· Overcoming barriers for families
What does a Case Manager do?
· Assesses the needs of the child or pregnant woman and their families
· Develops a plan to meet the needs
· Assists in locating services in the community to meet the needs
· Makes appropriate community referrals
· Provides follow up with families to assure needs are met
